Our Program
We aim to prevent gaps in services that often lead to unmet needs and isolated suffering among persons in crisis. To do this, we provide services directly to community service providers that help them access critical resources, as well as, linkages to other service providers to get what they need to be able to continue to help communities and be the bridge that ensures persons in need get connected to the resources they need instead of falling between the gaps.

C.A.P.
Join one of our C.A.P. meetings and connect with a diverse combination of community focused organizations, develop new collaborations with other organizations, find assistance for your organization and constituents, establish direct referral streams to prevent those in need from falling into the cracks of social systems.
